Amber Youth Wellness provides mental health outreach support for young people aged 12–25 years who are experiencing complex psychosocial challenges across Perth North.
Eligibility criteria:
- Young people aged 12–25 years
- Experiencing mental health concerns, including:
- 1b: Attenuated syndromes of severe mental disorders with moderate to severe functional impacts
- 2: First episode of severe mental health conditions with significant functional impacts
- Experiencing additional vulnerabilities such as:
- Low school attendance
- Social isolation
- Limited family support
- Housing stress
- Identifying as Aboriginal and/or Torres Strait Islander, CALD, or LGBTIQ+
- Residing within the Perth North PHN outreach area
