The Reconnect Program

The Reconnect Program provides outreach case management support for young people aged 12–18 years (or up to 21 years for newly arrived migrants) for a period of up to 6 months.

This program supports young people who are experiencing family conflict or are at risk of homelessness. Reconnect works closely with young people and their families to improve relationships, strengthen communication, and increase engagement with education, community, and support networks. It also assists those who may be disengaged from school or have limited community connections.

Eligibility criteria:

  • Young people aged 12–18 years (up to 21 for newly arrived migrants)
  • Young people who are not in the care of the State or CEO (due to funding arrangements)
  • Experiencing family conflict, disconnection, or risk of homelessness

Service details:

  • Provides outreach support to both young people and their families
  • Focuses on strengthening relationships and improving wellbeing
